Importing the SVG here into my file, http://openclipart.org/detail/20914/hea ... -baabullah
I then reduce the art to about 36x36 pixels and desaturate it to make it black & white. The clip art looks fine on screen at any zoom, but it becomes pixalized when printed out to paper or PDF.
I don't know what I have to do to make it look good on paper.

I think that pic is png file not an SVG, so it would print out rasterized most likely unless you print it out at exactly the same size the original file is.

Possibly. But it looks smooth even ridiculously zoomed in.
I didn't desaturize the image. And it printed fine.
You could be right though. Perhaps tomorrow I'll test it again.

Welcome to InkscapeForum!
If it printed fine when you didn't desaturate, maybe it was the desaturating that caused the problem. How did you desaturate? And also, I'm curious why you desaturated to achieve black and white, instead of just changing the fill to black.
