In this work the icons are from Windows Vista, but all the rest (background, buttons, reflections, etc. etc.) is pure Inkscape.

You've done a brilliant job.
Have you created skins with other software before using Inkscape? I had started creating a skin for Opera using Inkscape and I loved how you could export a selection. Previously I had created skins in Photoshop and it was such a chore to slice up the image. I think I prefer to use Inkscape.

I export transparent PNGs from Inkscape and composite them in Gimp layers for slicing up all the different "states".
This is mainly because the final images must be transparent TGA, but also because Inkscape takes into account blurs and other elements that make the export less "pixel-perfect".
A technique I used a bit is to create a "slices" layer on top of all; in this layer I created some simple rectangles - with red borders - for the slices I wanted to produce. After that, into Gimp, I removed the red border getting a precise selection.
Anyway this is one additional step over the simple export and import.

How do you mean? Inkscape doesn't blur anything you don't specifically choose to blur.

Yes, you are right. The fact is that there are many elements with Gaussian blur activated in these drawings, so the bounding box used for export is not so useful.

Oh, ok. When you say "pixel-perfect" you mean the size of the slices not the quality of the image. I understand.

well , it's a professional looking so perfect themes , i may reach your level ... in ... (how should i say it ?) well , in 100 years maybe ,
i have a word for slicing ... i was using fireworks for making webpage u design all the page and then select your cells then export all the image as a XHTML table with image ....
i miss this tool in inkscape ... i've found that inkscape has a brilliant way in editing the paths but after i design my page i have to export it to PNG and import in to GIMP and make guides (which i hate) and export the images along slices (that is a pain to me)
i really want inkscape to have this tool (slice rectangle) > export as XHTML
