I'm trying to import a PDF into Inkscape 0.46 on WinXP. The preview image shows up in the PDF import window, but after clicking "ok" it always displays a huge "linked image not found" in the size of the PDF page (A4).
Opening the PDF with Adobe Reader 8 works correctly.
Does anybody know why this happens and how I could get the PDF to load in Inkscape?

Are you able to provide a copy of the PDF so we can test it?
After opening it I would check in the XML editor to see if it reveals and issue with how the image is being loaded.

I also wonder how to import the included AI file correctly in Inkscape. If I use the thumbnail view for icons in WinXP, it shows me a full A4 site with 6 shoe templates. Once I import the file into Inkscape, the lower image is gone.

Well, the image in the PDF is just a PNG, so this is good for you.
Inkscape seems to have problems converting the image to a data URI. A data URI is basically a way to embed a binary file (i.e. a PNG) into a text file (i.e. HTML or SVG). If you deselect the option "Embed images" when you open the file, Inkscape will not use a data URI and will instead save a copy of the image in a temporary location. The file should open fine this way, but I think there might be some corruption in the original PNG.
Anyway, because there is no vector information in the PDF, I wouldn't even bother opening it in Inkscape. Instead I would open the PDF in Acrobat Reader and copy the image out of it to work with--this process should correct the corruption (it did for me).
As for the AI file, I don't have Illustrator, but you can open AI files in Acrobat Reader and Acrobat also doesn't show the sole. The preview you see in WinXP is only an embeded thumbnail--it's possible the thumbnail is out of sync with what the actual image is. Note that the AI file contains a true vector version of the template, so this is the file I would be working with.

... if you open the AI in Inkscape ungroup the drawing (you need to ungroup it twice) the other shoe template will appear.

Ah yes, for me it appeared after ungrouping 3 times. Thanks.
Now I could wonder why it is hidden when grouped, but I'm not going to do that as it's the anyway illogical Inkscape.

It's hidden when grouped because the group is clipped to exclude it. Ungrouping loses the clipping (though it shouldn't - this is a bug).
The normal way to remove the clipping would be to use the clip release command (Object -> Clip -> Release), and then delete the black clipping rectangle.

I'm not sure it is a bug, at least not with Inkscape. Acrobat displayed exactly what Inkscape displayed.

Yes, I meant that it's a bug that the clipping disappears when ungrouping (ungrouping shouldn't change appearance). It's not a bug that the clipping is present to start with, since the clipping is present in the AI file too.
